BATANGAS CITY - The Batangas city government has already started conducting a survey of its residents who want to get vaccinated against COVID-19.
The survey can be completed online by clicking the link https://docs.google.com/.../1FAIpQLSdnV7mDTVm.../viewform... on the Palakat Batangas City facebook page and instagram account.
Remote barangays that do not have or have poor internet will be given survey forms for their residents.
Some of the information that needs to be provided is the name, address, birthday, mobile number, and email address.
It is also necessary to answer the questions if you agree to be vaccinated and if not, you need to state the reason why.
It is also known if there is a type / type / brand of vaccine you want and what it is.
If another type / type / brand of vaccine comes to this city, the respondents are asked if they will get vaccinated again.
For the safety of the residents, it is also known if they have allergies to food, medicine, or vaccines and what they are.
